Monday February 27th First Day of Tryouts-Must have a Physical (on the offical Parkway Form. See link below for form) and Emergency Card

1st day of Tryouts
Need to have:
1. New Parent Permission Form (Emergency Card)
2. New Physical form on file (if it is in the nurses office, please have nurse notify Dave McFarland prior to tryouts to cinfirm that it is on file)
3. New Student Agreement
4. New Parkway Code of Conduct Form
